Maintain and/or improve residents’ ability to manage physical, mental, and psychosocial needs
Identify individual social and emotional needs
Develop and maintain individualized care plans
Participate in admission planning, community re-entry, and transfers
Attend interdisciplinary care plan meetings to support residents’ emotional and social well-being
Bachelor’s Degree in Human Services, Psychology, Social Work, or related field
MSW required
Long-term care or subacute experience required
Bilingual Chinese-speaking candidates preferred
