DESCRIPTION

Under general supervision of the Human Services Supervisor - Behavioral Health, this position provides Adult Mental Health Targeted Case Management (TCM) services to adults with serious and persistent mental illness, including individuals under court commitment for mental illness or chemical dependency. The Social Worker - Behavioral Health Case Manager supports the mission of Blue Earth County Human Services by connecting individuals to services that promote stability, recovery, and community participation.

Responsibilities include assessing client needs, developing person-centered service plans, coordinating and monitoring services, facilitating access to community resources, providing crisis support, and ensuring compliance with court-ordered services. This position also participates in discharge planning, monitors court commitments, and engages in ongoing professional development.

DUTIES / RESPONSIBILITIES

ESSENTIAL FUNCTIONS:

  • Provides Adult Mental Health Targeted Case Management Services in accordance with Minnesota Statutes and Department of Human Services guidelines. Meets with clients to assess strengths, service needs, and needed community supports. Develops person-centered case plan and goals. Coordinates and monitors the provision of services between clients and service providers to ensure services are being provided, utilized, and effectively meet the needs of clients. Provides case management services including assessment, planning, referral/linkage to community-based services and monitoring/service coordination. Opens, maintains, and closes case files. Case management services may include helping the client obtain needed mental health services, social services, educational services, health services, vocational services, recreational services, and related services in areas such as housing services, advocacy, transportation, and legal services. Travels to appointments within and outside of Blue Earth County as required.
  • Develops and updates Functional Assessments (FA), Individual Community Support Plans (ICSP), and General Service Plans in collaboration with clients, families, providers, and other stakeholders. Ensures plans reflect client needs and comply with program requirements.
  • Maintains accurate, timely documentation of client contacts, services, and activities. Completes required reports, data privacy forms, and releases of information. Assists with scheduling diagnostic assessments and tracking billable and non-billable activities.
  • Monitors court commitments and compliance with mental health and/or chemical health services and treatments ordered by the Court. Writes reports to the court as mandated by statute.
  • Participates in discharge planning by coordinating with clients and facility staff to identify support needs, develop discharge goals/plans, and facilitate referrals and appointments.
  • Provides crisis assistance by identifying and accessing additional support services during times of mental health crisis.
  • Promotes a diverse, culturally competent, and respectful workplace.
  • Engages positively with both internal and external customers. Communicates and interacts effectively and respectfully with colleagues, supervisors, representatives from other organizations, and the public.

  • Attends work during regularly scheduled hours.

ADDITIONAL FUNCTIONS:

  • Performs intake duties as assigned by obtaining information from clients to determine service needs. Assists clients to obtain information about local resources and provides referrals as needed.
  • Assists with completing pre-petition screening for court commitment as needed.
  • Participates in professional staff development. Participates in training and education opportunities to maintain and improve proficiency. Attends and participates in professional organizations. Associates with employees and departments from other counties and states for collaboration.

QUALIFICATIONS

MINIMUM QUALIFICATIONS:

  • Bachelors degree in social work, sociology, psychology, or related field from an accredited college or university; and
  • 2,000 hours of supervised experience in the delivery of services to adults with mental illness; or
  • Equivalent combination of education and experience necessary to perform the essential functions of the position.

PREFERRED QUALIFICATIONS:

  • A minimum of two (2) years of experience providing Adult Mental Health Targeted Case Management Services in a county human services agency.
  • Proficient in understanding and speaking languages other than English, such as Spanish, Somali, Chinese, Sudanese, and Arabic, among others.

CONDITIONS OF EMPLOYMENT:

  • Must successfully pass a background check.
  • Must possess a valid driver's license to drive in the State of Minnesota, reliable transportation, and the ability to travel to client homes and community locations as required.
  • Must comply with all federal and state laws and organizational and departmental policies and regulations.
  • Must adhere to data practices policies and standards regarding non-public data. Access to non-public data must be limited to what is necessary to perform job duties. Employees must take reasonable steps to prevent unauthorized access and ensure secure storage of data in accordance with county policies and Minnesota Statutes.

HOURS OF WORK

Monday through Friday from 8:00 a.m. to 5:00 p.m. This position allows for flexible hours and may require some evening and weekend hours. This position may require limited participation in an on-call rotation, which may include nights, weekends, and holidays to respond to urgent needs.

Blue Earth County features a 12-Step Pay Plan. Our advertised salary ranges include all 12 Steps. A selected candidate will typically be offered Step 1 of the salary range. Additional salary considerations are subject to negotiation and dependent on skills and experience directly related to the position.

Vacation Leave

Vacation accruals, utilization, and payouts are detailed in the collective bargaining agreement and/or the employee handbook.

Sick Leave

Employees accrue sick leave at a rate of 7.34 hours for each full month of service.
