If you are looking for a full-time position as a Social Worker in the field of Alzheimer's and Dementia this position is for you. This position is with Eddy Alzheimer's Services, a department within St. Peter's Healthcare Partners. We are a 100% New York State Department of Health, Caregiver Support Initiative Grant Funded Program - employment contingent on continued funding.

What you will do:
The Licensed Master Social Worker will provide advanced social work services, care planning, and community resource coordination/navigation to caregivers and families in the community caring for individuals with ADRD. Work involves the use of sophisticated interviewing, assessment, and psychotherapeutic interventions aimed at positively affecting the physical, cognitive, and emotional functioning of our clients. The Care Navigator will assist in identifying appropriate community resources and programs to support our clients and their families to maintain their independence in the community for as long as possible.

Requirements:
Master's Degree in Social Work is required.
NYS LMSW or Limited LMSW Permit eligible is required.
Minimum of 1 year of experience in the field of Alzheimer's/Dementia and demonstrated knowledge of the disease and challenges for caregivers is preferred.
Pay Range: $30.00 - $43.08
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
