Job Description:

  • Tracks and assures that state resource dollars remain within budgeted amounts specified by Plan of Care, and as indicated by client need.
  • Performs timely pre-screening evaluations for all referrals, addressing eligibility requirements, services needed, and perceived or actual limitations.
  • Assures clients' Medicaid status/eligibility, both initially and throughout the timeframe of service provision, coordinating home care, admissions, transfers, and discharges with appropriate care providers and business/community personnel.
  • Processes completed FL-2 forms for prior approval status, with appropriate level of care established.
  • Assesses and reassesses clients and existing services to determine initial and continued CAP participation.
  • Develops appropriate Plan of Care (POC for each client, utilizing local approval or communicating with DMA liaison, and adjusts POC as indicated by client's physical and/or environmental situations.
  • Ensures that care is provided and documented according to established Plan of Care, and that client/family is satisfied with services; monitors utilization and provision of services to assure optimal client success.
  • Prepares, maintains, and ensures that necessary records and reports are completed to DMA, DSS Medicaid standards.
  • Provides social work perspective for CAP/DA team when considering resource needs, services, and status of prospective or established clients.
  • Provides ongoing liaison activities between the client/family and the physician and his/her staff.
  • Other duties as assigned.

Knowledge, Skills, and Abilities:

  • Proven position people skills [Required]
  • Able to communicate effectively in English, both verbally and in writing [Required]
  • Knowledge of CAP programs and applicable Medicaid regulations [Preferred]
  • Additional languages [Preferred]

Education:

  • Bachelor's [Required]

Field of Study:

Work Experience:

  • Demonstrated social work experience in health care setting [Required]
