  • Position Type: Contract
  • Contract Duration: 3 months
  • Pay Rate: $67.00 / Hour (USD)
  • Shift / Schedule: Day (5x8 Hours), 8:00 AM 4:30 PM
  • Travel Requirements: Traveler is expected to have their own transportation. Occasional maximum radius of roughly 60 miles (60 90 minute total drive time) per day. Mileage Reimbursement: IRS Standard.
  • Work Authorization: Applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.

Job Summary

This is a Home Health LMSW contract position serving East Portland, OR and surrounding areas. The traveler must have both OR and WA licensure to be considered for this role. First-time travelers are accepted, and local candidates within a 50 75 mile radius are welcome at the same rate.

Key Responsibilities

  • Provide skilled home health social work services with a 1:1 patient ratio; may visit up to 6 patients per day with a standard of 30 productivity points.
  • Utilize EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/Oasis for charting.
  • Manage scheduling and travel efficiently within a service area; the system groups clients to minimize drive and windshield time, with an occasional maximum radius of roughly 60 miles (60 90 minute total drive time) per day.
  • No on-call requirements.
  • No weekend requirements.
  • No floating between like units required.

Required Skills

  • Oasis experience is required.
  • Ability to manage a caseload of up to 6 patients per day and meet 30 productivity points standard.
  • Own reliable transportation required.
  • Must have both OR and WA licensure (pending license accepted traveler must have both OR and WA licensure).

Education Requirements

  • Licensed Master of Social Worker (LMSW) both Oregon (OR) and Washington (WA) licensure required; pending license accepted provided both states are covered.
  • BCLS (Basic Cardiac Life Support) certification required.

Required Experience

  • Minimum 1 year of Skilled Home Health experience required.

Work Environment / Physical Requirements

  • Type of Facility: Home Health
  • Attire / Scrub Color: Business casual; no jeans. Scrubs allowed but not required (no specific color).
  • Charting: EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/Oasis.
  • Parking Cost: Free.
  • Facility cannot accommodate 4x10 or 3x12 shift schedules.
  • Block schedule not available.
  • Facility requires primary series COVID vaccination in addition to one booster.

Important Notes

  • Traveler must have BOTH OR and WA licensure this is a hard requirement. Pending license accepted provided both states are covered.
  • Locals accepted; considered "local" within a 50 75 mile radius and will be accepted at the same rate.
  • Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks).
  • RTO Restrictions: Confirm with hiring manager.
  • Open to first-time travelers: Yes.
