Shift: 5 days per week 08-1630 or 0730-1600 on an adult med surg unit at Missouri Baptist Medical center. Minimal weekends required (1 full weekend (Saturday and Sunday per 8 weeks; can work a split weekend to total 2 weekend

Special Requirements:
Required: short term acute care inpatient medical social worker, med surg unit, develop plan of care to meet patient/caregiver needs, coordinate with internal and external team members, document pertinent information to facilitate care coordination, participate in care conference activities, refers to rehab and long term care facilities, refers to other disciplines/resources, discharge summary, documentation according to plan of care, documentation of patient/caregiver teaching/learning, documents patient response, facilitate discharge and contingency planning, understanding and utilization of short-term therapy, educate patient/caregiver to facilitate discharge, provide/reinforce individualized patient/caregiver education, EPIC

Preferred: community hospital experience, biopsychosocial assessment, collect and analyze patient information, establish and revise goals, review of health history, collaborative care, coordinate services to home care and equipment following discharge, coordinate quality care n cost effective manner, documentation reflects understanding of coverage criteria, long-range planning and decision making, provide crisis intervention, Medicaid/Medicare, medical transportation, HCBS, Medicare waiver, VCP, adult protective services, power of attorney, advanced directives, ADA, high risk intervention patient vulnerability or safety risk, knowledge of cultural issues, patients with communication problems (hearing, speech), LCSW, CarePort
