QUALIFICATIONS

Bachelor's degree in Social Work or a related field and three (3) years of Case management experience w/ Mental Health and Co-Occurring Disorders population.

RESPONSIBILITIES

Primary Responsibilities:

- The Continuity of Care Caseworker is assigned a caseload and interviews their clients, gathers all pertinent information including previous or present treatment providers, and any criminal justice agencies, etc., provides individual and family education, maintains accurate and complete clinical records, and implements the discharge and referral process.

- The Continuity of Care Caseworker is a member of the interdisciplinary treatment team. The Caseworker brings information to the team, has input in the treatment and discharge planning, implements their part of the treatment programming and implements the discharge process.

- The Continuity of Care Caseworker ensures the quality of documentation contained within the record for each assigned patient and completes the following in the required format in a timely manner: and that the following is in order: Psychosocial history & Assessment, Confidential Release forms, Treatment Plan, Aftercare Plan, Educational Interventions, progress notes and other reports.

- Must have excellent oral and written communication skills, as well as be detail-oriented and able to complete a high volume of documentation in a fast-paced environment. Must be able to work independently and professionally with others.
