Pay Range: $18-22/hr
We are looking for candidates that have the aspiration to become a Social Services Director within the world of skilled nursing. We're looking for ambitious, resourceful, and confident individuals who enjoy working with patients, families, and team members.
Essential Functions: Provide support to Social Services Director and carry out designated assignments which may include:
• Assess patients’ psychosocial, financial, and support needs to develop safe and appropriate discharge plans
• Collaborate with physicians, nurses, therapists, and interdisciplinary teams to coordinate post-discharge care
• Arrange services such as home health care, rehabilitation, durable medical equipment, transportation, and community resources
• Educate patients and families on discharge instructions, treatment plans, and available support services
• Advocate for patients to ensure continuity of care and access to necessary resources
• Coordinate placements in skilled nursing facilities, assisted living, or other care settings when needed
• Maintain accurate and timely documentation in electronic medical records in compliance with regulatory standards
• Monitor discharge progress and adjust plans based on patient condition and resource availability
• Address social determinants of health, including housing, food insecurity, and access to care
• Participate in care conferences and discharge planning meetings
Other Specific Requirements
• Must provide evidence of being free of tuberculosis infection upon hire and as set forth by the policies of the facility.
• Must be able to read, write, speak and understand the English language.
• Supervisory and/or administrative experience in rehabilitative long term care preferred.
• Knowledge of the Medicare PPS system preferred.
• Must possess the ability to make independent decision when circumstances warrant such action.
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
• Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
• Follows written and oral instructions.
WORK ENVIRONMENT:
• Works in resident areas, office areas and throughout the facility.
• Moves intermittently during working hours.
• Is subject to frequent interruptions.
• Subject to odors and is exposed to dust, disinfectants, hazardous chemicals, tobacco smoke, and other air contaminants.
• Subject to hostile and emotionally upset residents, family members, personnel, visitors, or others.
• Subject to exposure to infectious waste and diseases.
