Bakersfield Post Acute is seeking a compassionate and motivated Social Services Assistant to join our team. General Purpose
The primary purpose of this position is to assist in planning, developing, organizing, implementing, and directing the facility's social service programs in accordance with federal, state, and local standards to ensure the emotional and social needs of each resident are met.
Essential Duties
Assist the Social Services Director in managing and coordinating social service programs.
Support residents in achieving the highest level of independence and psychosocial well-being.
Communicate with residents, families, and interdisciplinary teams regarding care planning and needs.
Provide information on Medicare, Medicaid, and other financial assistance programs.
Maintain accurate social service documentation and progress notes.
Assist with discharge planning, referrals, and coordination with outside agencies.
Help coordinate appointments and ancillary services (e.g., podiatry, dental, psychiatric).
Address emotional and adjustment concerns for residents and families.
Participate in care plan meetings, staff meetings, and in-service training.
Maintain confidentiality of all resident information and comply with HIPAA regulations.
Develop and maintain strong relationships with community resources and agencies.
Qualifications
Education & Experience
High school diploma or equivalent required
Associate or Bachelor's Degree preferred
Previous healthcare or skilled nursing experience preferred
Skills & Abilities
Strong communication and interpersonal skills
Ability to read, write, and interpret reports and policies
Organizational and problem-solving skills
Ability to work effectively with residents, families, and interdisciplinary teams
Physical Demands
Ability to sit, stand, walk, and use office equipment regularly
Occasional lifting up to 25 lbs
May assist with resident needs during emergencies
