Social Services Assistant
Location: Aiken Rehabilitation and Care Center | Aiken, SC
Schedule:Full-Time | Monday–Friday | 8:00 AM – 4:30 PM
Aiken Rehabilitation and Care Center is seeking a compassionate and dedicated Social Services Assistant to join our skilled nursing team. In this important role, you will support residents and their families by advocating for their social and emotional well-being while assisting in the coordination of the Social Services Department.
Responsibilities
- Assist in developing individualized care plans that address residents' psychosocial and emotional needs
- Support the admission and discharge process, ensuring smooth and supportive transitions
- Provide emotional support through individual and group interactions during periods of adjustment or crisis
- Maintain accurate, detailed documentation of residents' behavioral and psychosocial status
- Assist in coordinating volunteer programs and community engagement initiatives to enhance residents' quality of life
- Educate and orient residents and families on facility services, policies, and available resources
- Collaborate with the interdisciplinary team to ensure residents receive comprehensive, person-centered care
Qualifications
- Bachelor's degree in Social Work or a related field preferred
- Experience in long-term care or a healthcare setting preferred
- Knowledge of federal and state regulations governing social services in skilled nursing facilities
- Excellent communication, organizational, and interpersonal skills
- Compassionate, professional, and resident-focused approach to care
