About this Job

Jefferson Parish Human Services Authority (JPHSA) is a Local Governing Entity (LGE) created by the Louisiana Legislature in 1989. JPHSA is responsible for the management and operation of behavioral health and developmental disability services in Jefferson Parish. JPHSA provides fully integrated primary and behavioral healthcare services through its JeffCare program.

The Social Service Counselor 5-B position serves as Jefferson Parish Human Services Authoritys (JPHSA) Centralized Care Coordination (CCC) Supervisor/Contract Monitor. This position ensures referrals of individuals from external and internal sources are streamlined and efficient; that individuals served are directed to the proper service area(s); and, that referral sources are provided adequate and appropriate communication throughout the process, including follow-up. The incumbent also monitors contracts relevant for the purpose of Care Coordination and reports to the Mental Health Regional Director in Executive Administration.

Minimum Qualifications

Six years of social services experience; OR

Six years of full-time experience in any field plus three years of social services experience; OR

A bachelors degree plus three years of social services experience; OR

An advanced degree in a social sciences field plus two years of social services experience.

EXPERIENCE SUBSTITUTION:

Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

NECESSARY SPECIAL REQUIREMENT:

Positions that provide addictive disorder counseling or prevention services will require certification or eligibility for certification issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor.

Job Duties and Other Information

Job Duties:

As a Social Service Counselor 5B, you will:

  • Monitors and tracks all external and internal referrals documented in the Division Care Coordinator Tracking Tool.
  • Develops monitoring reports for presentation to the Executive Director, CPS Division Director, and Service Area Division Directors on a quarterly basis. Tracks trends in the numbers and types of internal and external referrals received, investigates fluctuations, and reports identified concerns to the Quality Improvement Specialist and Service Area Division Directors.
  • Works collaboratively with the Quality Improvement Specialist and outside consultant to develop an evidence-based practice (EBP) by attending scheduled meetings, tracking and compiling data, etc.
  • Distributes a satisfaction survey to external referral sources. Tracks responses and trends in feedback received, conducts follow-up activities as necessary, including scheduling in-person meetings with external stakeholders/referral sources as necessary to determine gaps or opportunities for improvement.
  • Attends external and internal stakeholder staff/outreach meetings, ensuring clear and up-to-date communication. Works in collaboration with other division supervisors as needed to ensure barriers to care are addressed.
  • Processes all new internal and external referrals and assigns them to Centralized Care Coordinators to ensure seamless linkage to services and appropriate follow-up for all individuals served within JPHSAs three (3) service area divisions: JeffCare, Behavioral Health Community Services (BHCS), and Developmental Disabilities Community Services (DDCS). Adheres to established CCC workflows when processing referrals by:
  • Completing necessary forms in the EHS (Pre-Admit, Internal Client Referral, JPHSA Referral Form, Client Document Capture, Parent/Guardian Information, Client Alert);
  • Uploading the external referral form and supportive documentation to the EHS;
  • Assigning accepted referrals to Centralized Care Coordinators in the EHS.
  • Provides direct supervision to all CCC staff (Centralized Care Coordinators) to ensure appropriate provision of care coordination services to all referred individuals and referral sources, per CCC procedure and established workflows.

Work Schedule: Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with some flexibility as needed to accommodate the needs of the individuals served.

Compensation: The salary offered will be determined based on qualifications and experience.
