Job Description

Join a compassionate care team at Hilo Benioff Medical Center where your work directly supports patients and families during critical transitions in care. As a Social Service Assistant, you will play a vital role in ensuring patients receive the resources, coordination, and support needed for a safe and successful discharge. This position makes a meaningful impact across the continuum of care, helping improve outcomes and patient experiences throughout East Hawaii. This position is a shared position between Rehab and Patient Services.

Key Responsibilities:

  • Collaborate with social workers, case managers, and multidisciplinary teams to support patient discharge planning
  • Coordinate follow-up appointments, durable medical equipment (DME), and post-hospital services
  • Assist with maintaining accurate and timely medical record documentation
  • Provide direct support to patients and families navigating post-hospital care needs
  • Communicate effectively with internal departments and external service providers to ensure continuity of care
  • Support patient care services across multiple settings, including acute care, long-term care, and outpatient programs
     

REQUIRED QUALIFICATIONS: To qualify, you must meet all of the following requirements. Please note that unless specifically indicated, the required education and experience may not be gained concurrently.  In addition, qualifying work experience is credited based on a 40-hour workweek.

Minimum Qualifications

Education & Experience:

  • High School diploma required
  • 3 years (IV level) - 4 years (V level) of experience providing supportive services to professional workers in social service or other health related agency
  • Knowledge of patient care coordination, discharge planning, or community resources


SUBSTITUTIONS ALLOWED:

  1. Successful completion of thirty (30) semester credit hours of coursework from an accredited college or university, may be substituted for experience on a year-for-year basis up to a maximum of three (3) years.
  2. Satisfactory completion of a human, social services, or health sciences curriculum at an accredited community college, technical school, or junior college which resulted in a Certificate of Achievement, Associate of Science, or related degree may be substituted for experience on a year-for-year basis, up to a maximum of two (2) years.

* Rates listed for paid holidays, vacation and sick days are for 1.0 Full Time Equivalent (FTE) positions assigned to 8 hour shifts. Accruals for these benefits are otherwise prorated based on FTE.

Pre-Employment Health Requirements

Offers of employment will be contingent on successfully passing a pre-employment examination, which includes a drug screen and other regulatory medical requirements such as, but not limited to, a tuberculosis (TB) screen.  The cost for drug screening and blood draw for QuantiFERON and Titers, shall be borne by the Hawaii Health Systems Corporation.

Shift Details
