Job Title: Social Care Network (SCN) Transportation Coordinator Location: Clifton Springs Employment Type: Full time Job Summary: In accordance with the organization’s mission, vision and values, the Social Care Network (SCN) Transportation Specialist is responsible for providing and coordinating transportation services that address health-related social needs (HRSNs), ensuring individuals can access medical, behavioral health, and community-based services. This role supports engagement and continuity of care by reducing transportation barriers, coordinating with care teams and community partners, and providing person-centered support during interactions. The Transportation Specialist also observes and communicates emerging needs, helping connect individuals to additional SCN services that promote stability, access, and improved health outcomes. Minimum Qualifications: High School Diploma or equivalent required. Relevant experience in human services, transportation, behavioral health, or working with underserved populations preferred. Ability to engage individuals in a person-centered manner and knowledge of community resources preferred. Experience supporting individuals with substance use, mental health, or chronic conditions preferred. CRPA certification preferred but not required; candidates with lived experience and willingness to pursue certification are encouraged to apply. Valid New York State Driver’s License with a safe driving record required.
