Youll work closely with a dedicated network of providers to connect members with vital HRSN (health related social needs) services, ensuring they receive continuous support and follow-up.

Minimum Education and Experience

• Associates degree in a human services field with one year experience in a human services position.
• The preferred candidate will have experience working with people with Intellectual and/or Developmental Disabilities, care management experience, and a working knowledge of community supports.

Licensure/Certification

• Valid NYS drivers license required.
• Must maintain current required certifications as designated by the agency including but not limited to: First Aid/CPR.

Pay Range The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, internal equity, and location.
