We are hiring for all office locations:
Note for San Andreas office: Your caseload location (Tuolumne, Amador, or Calaveras) will be based on where you reside.
What Youll Do:
Key Duties for the Service Coordinator Position:
Case Management & Planning
Assess, coordinate, and implement individual program plans through team processes
Schedule and facilitate various reviews and team meetings
Documentation & Reporting
Complete comprehensive case documentation including program plans, evaluations, progress reports
Prepare service authorizations, court reports, and Medicaid documentation
Advocacy & Support
Provide advocacy, education, and crisis intervention for clients and families
Serve as liaison between clients and various service providers
Data Collection & Analysis
Collect and report client statistical data for various surveys and assessments
Professional Collaboration
Participate in internal/external committees for education and agency improvement
Maintain positive communication with partner organizations
Emergency Response
Serve as Officer of the Day and participate in After Hours Response System as assigned
Areas of Support
This position may support individuals in the Children or Transition/Adults programs, depending on assignment and organizational needs.
