Service Coordinator – Elderly/Disabled

We are currently seeking compassionate and experienced candidates for the role of Service Coordinator – Elderly/Disabled in Akron, Ohio. This position is ideal for individuals who are passionate about supporting elderly and disabled populations and have a background in social services.

This role involves engaging directly with residents, assessing their needs, and coordinating services to help them maintain independence in their housing. You will work closely with community agencies, organize programs, and help residents build self-sufficiency. Hours:
08:00 AM – 04:30 PM, Monday – Friday

Pay:
$17.50/hour

Key Responsibilities:
  • Conduct resident interviews and in-home visits to assess needs and coordinate services
  • Develop case plans to assist residents in remaining independent
  • Enter accurate case notes and data into tracking systems (TAAG, Microsoft Office) for reporting and compliance
  • Promote and refer residents to community resources including transportation, healthcare, financial, and legal services
  • Coordinate on-site programs, workshops, and resident council meetings
  • Build and maintain partnerships with local community groups, social service agencies, and providers
  • Assist with recruiting individuals and families for Elderly/Disabled Services and self-sufficiency programs
  • Ensure compliance with HUD regulations, program guidelines, and confidentiality standards

Minimum Requirements:
  • Experience in social services and knowledge of resources for elderly and disabled individuals
  • 3–5 years of experience working with elderly or mentally/physically disabled populations
  • Strong knowledge of community resources and social service programs
  • Excellent data entry, communication, case management, and organizational skills
  • Ability to perform in-home needs assessments
  • Valid Ohio Driver’s License and insurable under company policy
  • Proficiency in Microsoft Office; experience with TAAG or similar systems a plus
  • Knowledge of HUD rules and regulations related to public housing (preferred)
  • LSW certification (preferred)

Nice to Have Skills:
  • Experience working within HUD guidelines
  • Strong interpersonal skills and ability to partner with community organizations
  • High standards of performance and excellent attendance

Preferred Education and Experience:
  • Relevant social services or community support experience
  • Experience working with elderly and disabled populations

Other Requirements:
  • Ability to work on-site at assigned properties and at the AMHA Central Office
