Schedule:  Typical shift is Monday to Friday days, however, must be flexible to working scheduled nights and weekends on occasion. 

Job Summary and Qualifications

Chippenham Hospital is seeking a self-motivated nursing professional with proven leadership experience to join our amazing Quality team as a Sepsis Coordinator Registered Nurse RN. What you will do in this role:

As a Sepsis Coordinator Registered Nurse you will be responsible for all programmatic responsibilities, as follows:

  • Ensures the evidence based screening and treatment protocols become the standard of care within the facility.
  • Collaborates with medical and hospital staff to provide multidisciplinary care to patients with a diagnosis of sepsis, severe sepsis and septic shock.
  • Reviews and analyzes clinical data and presents the data in an understandable and meaningful context to staff and physicians to motivate adherence to protocols.
  • Observes clinical processes and conducts surveillance activities, with immediate intervention (just-in-time) as indicated.
  • Forms idea for process improvement, develops and engages team solutions.
  • Provides education and clinical expertise to care team (physicians, LIPs and staff), patients and families.
  • Collaborates with physicians and staff in an effort to maximize timely evidence based care to decrease morbidity, mortality, length of stay, and readmissions.
  • Leads sepsis committee meetings with physician and nursing champions.
  • Leads hospital, medical staff and shared governance meetings focusing on sepsis as needed.
  • Participates in multidisciplinary rounds and proactively responds to patient needs to improve patient outcomes and positively impact overall patient experience.
  • Works in collaboration with the care team to develop and implement individualized teaching plans for patients and families.
  • Collects and analyzes sepsis program data to drive the program.
  • Serves as a liaison between physicians and clinical documentation improvement (CDI) specialists relative to sepsis diagnoses.
  • Participates in hospital, division and corporate wide collaboration for sepsis care and treatment.
  • Maintain knowledge and understanding of changing performance measurements for high quality patient outcomes.
  • Locates, accesses, and understands local and national best practice benchmarks related to patient experience and outcomes.
  • Interacts with patients/families regarding experience. For less than expected care experience, facilitates action and resolution with leadership, and provides timely written correspondence for case closure.
  • Adjustment in assignments may occur based on patient and hospital need, changing outcomes and metrics.

What qualifications you will need:
  • Graduate of an accredited School of Nursing required (BSN and/or MSN preferred).
  • Current Registered Nurse License in Virginia or Current Multistate Compact License required.
  • 3 years of current nursing experience in an acute care hospital setting required.
  • 1 year of recent ICU or Emergency Department nursing experience or Nursing Education (preferred).
  • Current American Red Cross or American Heart Association Basic Life Support (BLS or BCLS) Certification required.
  • Advanced Cardiovascular Life Support (ACLS) Certification (preferred; may be obtained within 60 days of position start date).
  • CPHQ (preferred; may be obtained within one year of position start date).
