Senior LTSS Service Care Manager (RN)

Summary:

• Work Mode: Remote & Field Visits (Laredo, TX)
• Duration: 6 months Contract
• Shift: M-F 8-5 with possibility for OT and Saturdays

Responsibilities:

• Identify needs of new members and continue care initiated by prior MCO.
• Prepare for visits by gathering information on members using our Electronic Medical Record system, reviewing diagnosis, medications, and recent authorizations.
• Complete visits that include Initial, Change in condition, MDCP, and Level 3 reassessments.
• Travel to member homes for in-person visits lasting 2-4 hours; travel can be up to 2 hours one way.
• Submit assessments within 24 hours. Initiate any service requests within 3 business days. Complete all other documentation within 5 business days.
• Hand off members to the appropriate level for assignment to a permanent care manager/service coordinator after 15 business days.
• Complete any task assignments within turnaround time per work process.

Requirements:

• Education/Certification: RN ASN, BSN, or MSN
• Licensure: RN
• More than 3 years as a nurse and 2 years of case management experience
