• Participates on EHR committees and message boards, acts as an advocate for improved hospice EHR.
• Assures timely and accurate completion of tasks required to meet regulatory requirement changes.
• Develops, builds, and maintains EHR forms, notes, visits, and pathways to reflect current needs and regulatory requirements.
• Assesses education needs, skills, competencies, behaviors and outcomes required for staff to improve performance, meet strategic objectives and achieve desired results. Develops plan to address needs uncovered.
• Provides training in EHR processes relevant to team members work so they can continue to provide optimal, efficient care.
• Assesses overall effectiveness of orientation and education/training programs. Designs methodology to obtain regular feedback from participants.
• Develops standardized written teaching materials for clinicians and internal team members including tablet training manuals and how-to- guides.
• Analyzes and reviews EHR database to develop improved documentation flow for clinical care and regulatory compliance.
• Acts as a resource to clinicians for EHR documentation and training. Is a superuser of EHR.
• Identifies and implements special or new quality improvement activities regarding EHR or management of equipment as agency needs are identified.
• Keeps up to date on the latest issues and trends in systems requirements an enhancements for hospice, through networking, professional memberships, and ongoing EHR education.
• Synthesizes trends and determines application and integration into department. Oversees the development of EHR educational programs that address these trends.
• Coordinates and collaborates with third-party vendor activities related to EHR and equipment management and activities.
• Coordinates and collaborates with other VNS service lines related to EHR.
• Reviews documentation regarding software updates, plans for and assures completion of education and implementation of process changes to take full advantage of capacity and operations of software applications.
• Recognizes and identifies ongoing needs for professional growth and development and assists Hospice leadership to train staff in the EHR systems in meeting needs.
• Tracks data related to employee competence and use of standard best practice protocols and reports results to Director of Hospice QAPI for performance improvement project.
• Takes the lead on implementation education of new software in the field.
• Assures all regulatory and compliance updates are implemented by required dates.
• Performs all duties inherent in a supervisory role. Ensures effective and ongoing business training and development of staff. Evaluates staff performance, provides input for the development of the department budget, and hires, orients, promotes and terminates staff and recommends salary actions, as appropriate.
• Participates in special projects and performs other duties as assigned.
Qualifications:Licenses and Certifications:
Home Care Home Base (HCHB) Coursework required
Education:
Bachelor's Degree in Health and Human services, Public Health, Computer Science or related field required
Work Experience:
Minimum of five (5) years’ experience in working with clinicians for EHR support required
Minimum of seven (7) years’ experience in health care with a minimum of three (3) years of focus in Hospice required
Knowledge of HCHB operating system for hospice is required
Pathway building access preferred
Knowledge of other community based software systems such as SWIFT, HIPAA messaging preferred
Practical knowledge in leading and managing the execution of processes, projects and tactics, and of hospice industry practices and standards preferred
