Description
The County of San Mateos Family Health Services Department is seeking a Senior Community Program Specialist to join our team! Family Health Services (FHS) is a division within San Mateo County Health that serves primarily families and children working towards their goals of improved health and brighter futures. This position is in the California Children's Services (CCS) Program.The Senior Community Program Specialist will play an integral role in supporting the development, design, and implementation of a new CalAIM program called Community Supports. This position will also support building a billing infrastructure into an Electronic Health System to create a more efficient and streamlined billing method and encounter reporting for the Enhanced Care Management and Community Supports program, while also assisting in the Claims process to ensure optimal and accurate reimbursement from the Payer. This position will be responsible for the Community Supports outreach and delivery and will also receive and process the CS Caregiver invoices on the back end to support the new program and a new process. This role will have analytic responsibilities involving quality improvement, establishing new efficient and innovative systems and processes, and supporting the overall establishment and expansion of CalAIM within California Children's Services.The Family Health Services Senior Community Program Specialist is expected to advance the goals and priorities of California Children's Services- Enhanced Care Management and Community Supports program through management of multiple projects and improvement efforts. The position will report to the Senior Community Program Supervisor and will work closely with the CCS Clinical Services Manager on multiple quality improvement projects.

Note: This is a limited term, at-will position. Limited Term Employees are assigned to assignments not to exceed three years. They receive medical and dental benefits, similar to regular employees, and a defined contribution retirement plan.
