Reports to: School Principal/Pupil Personnel Director

Job Goal: To maintain maximum health standards in compliance with Massachusetts State Health Regulations established for public schools.

Performance Responsibilities:

  1. Conducts assessments (i.e. physical assessment, health screenings, substance use, nutritional, environmental).
  2. Health Screening results are reported according to district and state requirements (i.e. parents/guardians, school personnel, physicians, hospital and clinics).
  3. Utilizes (electronic health record) system appropriately to document individual or group assessments and outcomes.
  4. Maintains current immunization records and keeps health records up to date.
  5. Assumes responsibility for identification and referral of students in need of medical and dental care.
  6. Follows up on screening referrals in a timely manner and documents completed referrals in student EHR.
  7. Participates in Student Support Team (SST), IEP and 504 meetings and makes appropriate recommendations for 504s or IEPs based on student health data and needs.
  8. Demonstrates epi-pen administration, proper use of an inhaler and nebulizers, and testing of blood glucose etc. to students and/or staff (observations and documentation).
  9. Provides meaningful input for creating individual student reentry plans, implementing short/long term academic accommodations regarding concussion regulations and/or, new chronic diagnosis.
  10. Addresses all students with respect and provides confidential space for students when necessary.
  11. Protects privacy and confidentiality of personal health information of students and staff.
  12. Assumes authority, in the absence of a physician, for the care of a student or staff member who has suffered injury or emergency illness
  13. Compiles and uploads to ASPEN a list of students with any vital health information such as medical condition and/or medication to all staff members.
  14. Prepares and submits health and medical reports requested by the school department and the State Board of Public Health.
  15. Assumes responsibility relating to exclusion of students in connection with infectious and contagious diseases and school entrance guidelines.
  16. Address school staff members on student health matters, particularly regarding screening for student health and medical conditions.
  17. Maintains record of CEU certificates and education programs attended.
  18. Maintains sufficient PDPs and CEUs for renewal of both RN and ESE licensure
  19. Shares information learned at continuing education programs with colleagues

Evaluation: Performance of this job will be evaluated annually, in accordance with the DESE Educator Evaluation System.
