Description

CHARACTERISTICS OF THE WORK

Under supervision from the FHC Chief Nursing Officer and with consultation from the FHC Chief Administrative Officer, performs program coordination, including staffing, clinical services, and program functions, at Family Health Centers (FHC) school-based clinics.

Serves as the School-Based Services Program point of contact for all FHC personnel and the key liaison between school leadership, health center providers, departmental directors, clinical personnel, and support personnel.

Responsible for ensuring efficient daily operations, coordination of health and supportive services, and consistent student-centered care that aligns with FHC and school district expectations, as well as State of Kentucky and Health Resources and Service Administration (HRSA) guidelines.

Examples of Duties

Assists with establishing, supporting, and maintaining interdepartmental communication regarding staffing, clinic needs, technical support, delegation of duties, and other matters within the clinics.

Supervises and coordinates the delivery of primary care, behavioral health, and dental services at school-based clinics.

Develops and implements quality improvement plans for efficiency and effectiveness.

Cooperates with stakeholders to facilitate services to adolescents, their families, school personnel, and the community.

Assists with onboarding new personnel.

Assists with creating training modules for school-based personnel.

Attends in-service trainings with school-based personnel.

Tracks annual credentialing, training, and skills completion needed for school-based personnel.

Completes or assigns personnel to complete timecards and record keeping.

Participates in annual performance reviews of school-based personnel.

Orders or assigns personnel to order supplies for school-based clinics.

Completes or assigns personnel to complete required safety/equipment checks.

Coordinates or assigns personnel to act as Vaccines for Children (VFC) backup coordinator to ensure correct vaccine inventories.

Assists with conducting internal assessments or audits to ensure readiness for operational surveys by external governing bodies.

Conducts research, analyzes data, and presents reports.

Travels between satellite clinics as needed.

Adheres to standard precautions.

Performs related work as required.

Minimum Requirements

MINIMUM REQUIREMENTS

Bachelor's degree in Public Health or a closely related field OR Associate's degree in Nursing.

Three years of experience working in the public health sector, including, but not limited to, a community health center, a public health department, or a targeted public health outreach initiative.

An equivalent combination of training and experience may be substituted as determined applicable by Civil Service.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

Responds to Family Health Centers' needs via the telephone, internet, fax, computer software and in person.

SPECIAL REQUIREMENTS

Must be available to work a varied schedule.

Must provide own transportation to work at other FHC locations as needed and on short notice.

Must obtain CPR certification by the end of the probationary period and maintain certification throughout employment with FHC.

Must be vaccinated for Hepatitis B, provide evidence of immunization, or sign a declination form within 10 days of initial assignment.

Must wear protective equipment as mandated by OSHA.
