We are currently seeking an experienced LVN Scheduler to join our Stockton team. This role is ideal for someone who thrives in a fast-paced healthcare environment, enjoys coordinating care, and takes pride in helping operations run smoothly behind the scenes.
What You’ll Do
- Coordinate and manage clinician schedules for patient home visits
- Utilize Homecare Homebase (HCHB) to create and maintain weekly scheduling
- Communicate with clinicians, patients, and leadership regarding scheduling needs and changes
- Assist with coordination of patient visits to support continuity of care
- Provide administrative support including data entry, filing, correspondence, and reporting
- Answer incoming calls and route messages professionally and efficiently
- Maintain accurate scheduling and patient records
- Support branch operations and assist with additional office duties as needed
Who We’re Looking For
You are organized, proactive, detail-oriented, and able to manage multiple priorities in a dynamic healthcare setting. You enjoy problem-solving, supporting clinicians, and helping ensure patients receive timely care.
Qualifications
- Active California LVN license required
- Previous home health scheduling experience required
- Homecare Homebase (HCHB / PointCare) experience preferred
- Strong communication and organizational skills
- Ability to multitask and adapt in a fast-paced office environment
- Professional and compassionate demeanor
- Proficient computer and administrative skills
