Responsibilities/Job Description:
Fairview is seeking a Same Day Nurse Practitioner to join our primary care practice at M Health Fairview Clinics – Burnsville. The Same Day Nurse Practitioner will provide full-scope care for same-day patient needs, including acute and chronic condition management and in-office procedures, as appropriate. Also provides cross-coverage for provider in-baskets during absences, with dedicated schedule time allocated. Practice Details:
- 15-provider primary care practice
- Full scope patient care
- Average of 18 patients per day
- FTE = 34 patient contact hours per week
- Openings: 0.75 to 1.0 FTE (Full Time) - Flexible schedule
- As part of Fairview’s Accountable Care Organization (ACO), providers practice in a collaborative, team-based model focused on quality outcomes, care coordination, and population health management.

- M Health Fairview Clinic – Burnsville is adjacent to Fairview Ridges Hospital.
Primary Responsibilities:
- Assess, diagnose, and manage acute and chronic conditions
- Perform histories and physical exams
- Order and interpret diagnostics
- Develop and manage care plans
- Prescribe medications
- Patient education and communication
- Documentation and follow-up
- Collaboration with care team
- Referrals and care coordination

Qualifications:
- Graduate of an accredited school of nursing and Nurse Practitioner training program.
- Board certified or board eligible as a Nurse Practitioner through AANP or ANCC
- Licensed or eligible for licensure as a Registered Nurse and Advanced Practice Registered Nurse (APRN) in Minnesota.
- Ability to obtain and maintain DEA certification in the state of Minnesota.
- Must meet credentialing and privileging requiremens.
