At The Springs at Sherwood, we're looking for a compassionate, organized, and resident-focused Memory Care Coordinator to lead our Memory Care team. In this role, you'll support caregivers, collaborate with families and healthcare professionals, and ensure every resident receives exceptional, person-centered care.

Position Details

  • Full-Time, Exempt (Salary)
  • Leadership position within our Health Services Department
  • Weekend and on-call availability required as needed

Pay Range: $57,000 - $66,400

What Does a Memory Care Coordinator Do?

As the Memory Care Coordinator, you'll oversee the day-to-day operations of our Memory Care neighborhood while supporting residents, families, and caregivers. Responsibilities include, but are not limited to:

  • Supervising and supporting the Memory Care caregiving team through coaching, mentoring, and performance management.
  • Ensuring residents receive compassionate, individualized care that reflects their changing needs and preferences.
  • Developing, updating, and maintaining resident service plans, temporary service plans, and care documentation.
  • Serving as a primary liaison between residents, families, physicians, and the care team.
  • Coordinating quarterly Care Conferences and collaborating with families to provide exceptional resident-centered care.
  • Assisting with hiring and ongoing staff training.
  • Monitoring staffing needs, assisting with scheduling, and providing floor support when necessary.
  • Reviewing resident changes, incident reports, and quality assurance measures to ensure regulatory compliance and excellent outcomes.
  • Coordinating new resident move-ins and helping create a smooth, welcoming transition into Memory Care.
  • Managing attendance, scheduling, and other administrative functions for the department.
  • Working closely with nurses, leadership, and interdisciplinary teams to promote the highest quality of life for every resident.

Qualifications

  • 3-5 years of experience in senior living, healthcare, or a related field.
  • Previous leadership or supervisory experience preferred.
  • Knowledge of Assisted Living and Memory Care regulations preferred.
  • Current CPR/First Aid Certification (or ability to obtain).
  • Strong communication, organization, and problem-solving skills.
  • Comfortable using Microsoft Office and electronic health record systems.
  • Ability to coach, mentor, and lead a team with compassion and professionalism.
  • Ability to pass a pre-employment criminal background check and drug screening.
  • A genuine passion for serving older adults living with dementia.
