POSITION SUMMARY

We are pleased to offer the opportunity to work as a a RN Wound/Ostomy Coordinator within the Kenneth Moy Care Center. This position supports the facility by providing professional nursing treatment and care to residents according to established rules, regulations and standard practices of the profession.

RESPONSIBILITIES

  • Assists in identifying patients at risk for skin integrity problems and facilitates the creation of appropriate plans of care.
  • Optimizes the resident wound/ostomy/continence experience across the Continuum of care
  • Works collaboratively with Care Center clinicians, physicians, nurses, and nursing leaders
  • Collaborates with MDS coordinators, Restorative staff, Physical Rehabilitation department, dining services, dietitian and others as it relates to wound ostomy, and incontinence management
  • Provides treatment within the scope of practice of a certified wound and ostomy nurse
  • Facilitates the referral process to expedite resident care across the continuum
  • Leads performance improvement efforts in preventing and reducing acquired pressure injuries
  • Builds effective working relationships with nursing leadership, advanced practice nurses, nursing staff, clinical resource management staff, and physicians
  • Organizes interdisciplinary wound rounds
  • Oversees and monitors specialty bed and bed/chair surface utilization
  • Provides education around wound, ostomy, and continence care to staff
  • Ensures timely and complete pressure injury data collection, entry and reporting to the Director of Nursing or other designated staff as needed
  • Supports IDPH survey readiness as it relates to wound, ostomy, and incontinence management
  • Responds to pressure injury queries from IDPH surveyors or risk management
  • Performs Root Cause Analysis or acquired pressure ulcers and develops performance improvements plan (PIP) in collaboration with Director of Nursing and other designated staff as needed
  • Prepares report on residents for next shift

REQUIREMENTS INCLUDE

Requirements include the following experience or an equivalent combination of training and experience:

  • Completion of coursework for a diploma in Nursing from an accredited college or university
  • Three(3) years of experience in acute and/or subacute settings including one year of inpatient or outpatient wound/ostomy experience
  • Possession of a current license in Illinois as a Professional Registered Nurse (RN)
  • Basic Life Support certification from the American Heart Association
  • Certification as a Certified Wound Ostomy Continence Nurse (CWOCN), Certified Wound Ostomy Nurse (CWON) or Certified Wound Care Nurse (CWCN) is required within six months of hire.

A post offer physical is required.

A pre-employment background check is required.
