Territory Clinical Manager (Home Health)

Location: Melbourne, FL
Employment Type: Direct Hire | Full-Time
Schedule: Monday–Friday | 8:00 AM–5:00 PM (Office-Based with Territory Travel)
Compensation: $90,000–$100,000 annually
Sign-On Bonus: Available

Position Overview

Wheeler Staffing Partners is seeking an experienced Territory Clinical Manager to provide clinical leadership, operational oversight, and regulatory compliance support for a skilled Medicare home health territory in Melbourne, Florida. This role is responsible for overseeing the delivery of high-quality home health services, ensuring compliance with Medicare regulations, supporting clinical staff development, and driving quality patient care outcomes.

The ideal candidate is a Florida-licensed Registered Nurse with skilled Medicare home health experience, strong leadership abilities, and expertise in OASIS documentation, quality improvement, and regulatory compliance.

Key Responsibilities

Clinical Leadership & Staff Management

  • Provide day-to-day leadership and support to clinical staff, including Registered Nurses, Licensed Practical Nurses, Therapists, and other healthcare professionals

  • Supervise, mentor, coach, and develop clinicians to ensure exceptional patient care and professional growth

  • Assist with onboarding, training, competency assessments, and performance evaluations

  • Support staffing, scheduling, and productivity management across the territory

  • Serve as a clinical resource and escalation point for patient care concerns

  • Foster a collaborative and patient-centered culture throughout the team

Quality & Regulatory Compliance

  • Ensure compliance with Medicare Conditions of Participation (CoPs), state regulations, accreditation standards, and organizational policies

  • Monitor clinical documentation for accuracy, timeliness, and regulatory compliance

  • Support survey readiness and regulatory audits

  • Lead quality improvement initiatives focused on patient outcomes and operational performance

  • Monitor quality measures, patient satisfaction scores, and CMS Star Ratings

  • Implement corrective action plans as needed to maintain compliance and performance standards

OASIS & Documentation Oversight

  • Provide guidance and oversight related to OASIS documentation and compliance

  • Support completion and review of:

    • Start of Care (SOC)

    • Resumption of Care (ROC)

    • Recertifications

    • Transfers

    • Discharges

  • Ensure OASIS accuracy to support quality outcomes, reimbursement, and compliance

  • Educate clinicians on documentation best practices and Medicare requirements

Clinical Operations

  • Support efficient delivery of skilled home health services throughout the assigned territory

  • Collaborate with interdisciplinary teams including nursing, physical therapy, occupational therapy, speech therapy, and social work

  • Monitor caseload management, visit utilization, and clinician productivity

  • Assist with patient care coordination and issue resolution

  • Support achievement of clinical, operational, and patient care goals

Community & Territory Support

  • Travel throughout the assigned territory to support clinicians and branch operations

  • Build relationships with referral sources, physicians, hospitals, and community partners

  • Participate in case conferences, leadership meetings, and operational initiatives

  • Serve as a representative of the organization within the local healthcare community

Required Qualifications

Education

  • Graduate of an accredited school of nursing

Licensure & Certifications

  • Active and unrestricted Florida Registered Nurse (RN) license required

  • Current CPR certification required

  • Valid driver's license, reliable transportation, and proof of automobile insurance required

Experience

  • Minimum 1 year of skilled Medicare home health experience required

  • Minimum 1 year of supervisory, management, leadership, or lead clinician experience required

  • Experience with OASIS documentation and Medicare home health regulations required

  • Ability to travel throughout the assigned territory required

Knowledge, Skills & Abilities

  • Strong knowledge of Medicare Conditions of Participation (CoPs)

  • Thorough understanding of home health regulations and compliance requirements

  • Knowledge of Medicare reimbursement methodologies

  • Understanding of quality measures, CMS Star Ratings, and performance improvement initiatives

  • Experience supporting survey readiness and regulatory compliance activities

  • Knowledge of skilled nursing, physical therapy, occupational therapy, and speech therapy services within home health

  • Excellent leadership, communication, organizational, and problem-solving skills

  • Ability to manage multiple priorities in a fast-paced healthcare environment

Preferred Qualifications

  • 3–5 years of Registered Nurse experience

  • Previous experience as a Clinical Manager, Assistant Clinical Manager, Patient Care Manager, Team Manager, or Home Health Leader

  • Experience managing interdisciplinary home health teams

  • Proven success improving quality outcomes, compliance metrics, and operational performance
