POSITION TITLE: RN Team Manager
REPORTS TO: Patient Care Coordinator
FSLA: Non-exempt

POSITION SCOPE:

  • Perform assessment of assigned patients and derive nursing diagnoses based on assessment data.
  • Plan nursing interventions in collaboration with the interdisciplinary team to resolve current problems or prevent potential problems.
  • Provide teaching to patients/families to assist in the patient's comfort as needed.
  • Provide emotional support to patients/families in a manner that fosters independence and affirms existing strengths.
  • Assess the need for home health aide services.
  • Assess and provide pain management strategies to assist with the patient's comfort.
  • Promote working in a team environment.
  • Provides management of team to ensure continuity of care for Hospice patients and families.
  • Comply with policies and procedures of Hospice of Huntington, Inc.
  • Protects the confidentiality of private health care information of patients
  • Promote the philosophy and services of Hospice of Huntington, Inc.

POSITION QUALIFICATIONS:

  • Graduate of an accredited professional school of nursing; BSN preferred.
  • Licensed registered nurse in West Virginia and Ohio.
  • Minimum of 2 years registered nursing experience; home health or hospice nursing preferred.
  • Proof of liability insurance.
  • Current driver's license, automobile insurance, and reliable transportation.
  • Philosophy compatible with the hospice concept.
  • Demonstrates an understanding of the various regulatory guidelines (ethical, confidentiality, legal requirements, etc.).
  • Possesses basic physical assessment skills.
  • Demonstrates a working knowledge of the stages of uncomplicated grief and the dying process. Calculates pain
  • Calculates pain medication doses using equianalgesic conversion tables.
  • Demonstrates proficiency in infusion management, including SQ, peripheral IV, and central venous routes.
  • Demonstrates proficiency in phlebotomy skills and in peripheral access device placement, including implementation of universal precautions and safe handling of sharps.
  • Possesses interpersonal skills that promote effective communication and interdisciplinary collaboration. effective use of
