We are dedicated to providing compassionate in-home care to clients and supporting caregivers and home health aides. As an RN Supervisor, you will play a pivotal role in ensuring the highest standards of patient care and supporting our team of caregivers across Albany, Saratoga Springs, Schenectady, and Rensselaer.


Key responsibilities include:

  • Conducting in-home client assessments and supervisory visits to ensure quality care delivery
  • Developing and reviewing individualized care plans for clients
  • Providing case management oversight and maintaining quality assurance standards
  • Supervising and supporting home health aides and caregivers in their daily responsibilities
  • Maintaining accurate documentation and ensuring compliance with regulatory requirements
  • Collaborating with the office team to coordinate care and resolve client needs
  • Participating in occasional on-call shifts, with additional compensation

Requirements

Required Qualifications:

  • Current New York State Registered Nurse (RN) license
  • At least 1 year of recent nursing experience
  • English proficiency
  • Authorized to work in the United States without visa sponsorship
  • Ability to travel locally for in-home visits approximately 30% of the time
  • Availability for occasional on-call shifts

Preferred Qualifications:

  • Experience in homecare or home health
  • Experience supervising or supporting home health aides or caregivers
  • Experience using HHAeXchange
  • Experience developing and reviewing patient care plans
  • Experience with nursing documentation and regulatory compliance in a clinical or home health setting
  • Experience conducting in-home patient/client assessments or supervisory visits
