POSITION SUMMARY

The RN Supervisor is responsible for overseeing clinical operations within the Assisted Living and Memory Support community, ensuring compliance with Texas Health and Human Services (HHS) Assisted Living Type B regulations. This role provides direct supervision of care staff, executes and monitors RN delegation, ensures safe medication management practices, and supports high-quality resident-centered care.

The RN Supervisor works under the direction of the Director of Assisted Living/Memory Support and serves as a clinical leader, educator, and regulatory resource for the department.

Key Responsibilities

1. RN Delegation & Clinical Oversight

  • Perform and document RN delegation in accordance with Texas Board of Nursing (BON) rules and HHS regulations.
  • Assess residents to determine appropriateness for delegation and identify tasks eligible for delegation.
  • Develop, implement, and maintain delegation plans, including:
  • Task-specific instructions
  •  Required competencies
  • Supervision schedules
  • Provide ongoing supervision and evaluation of delegated tasks to unlicensed personnel.
  • Ensure compliance with Rule 225 (RN Delegation) and applicable Assisted Living regulations.
  • Reassess residents regularly and update delegation as condition changes.
  • Maintain accurate and survey-ready documentation for all delegated tasks.

2. Medication Oversight & Management

  •  Oversee medication administration systems to ensure compliance with Texas Assisted Living licensing standards.
  • Monitor medication aides and direct care staff for safe medication practices.
  •  Conduct routine medication pass observations and audits.
  •  Ensure proper:
  • Medication storage and security
  • Documentation (MAR accuracy)
  • Handling of controlled substances
  • Collaborate with pharmacy providers and physicians regarding medication changes and discrepancies.
  • Investigate and document medication errors; implement corrective action plans.

3. Service Plan Development & Management

  • Participate in initial and ongoing resident assessments.
  • Develop, review, and update Individualized Service Plans (ISPs) based on resident needs, physician orders, and regulatory requirements.
  • Ensure service plans reflect:

          o   Functional abilities

          o   Cognitive status

          o   Behavioral needs (Memory Support)

          o   Nursing and personal care requirements

  • Conduct regular service plan reviews and interdisciplinary meetings.
  • Ensure care staff are trained and adhere to service plan directives.

4. Staff Training & Development

  • Provide initial and ongoing training to direct care staff, including:

        o   Delegated nursing tasks

        o   Medication administration

        o   Infection control

  • Validate competencies for all delegated tasks and clinical procedures.
  • Mentor and coach staff to improve clinical performance and regulatory compliance.
  • Participate in onboarding and orientation of new hires.
  • Identify performance gaps and implement training interventions.

5. Regulatory Compliance & Quality Assurance

  • Ensure compliance with Texas HHS Assisted Living Type B regulations and all applicable standards.
  • Maintain survey readiness at all times.
  • Participate in state surveys, audits, and inspections.
  • Assist in developing and implementing plans of correction.
  • Conduct routine clinical audits, including:
  • Resident records
  • Medication administration
  • Delegation documentation
  • Monitor incident reports and ensure proper follow-up and documentation.

6. Resident Care & Clinical Support

  • Provide direct nursing support as needed, including assessments and clinical interventions.
  • Monitor residents with complex medical needs common in Type B and Memory Support settings.
  • Collaborate with physicians, families, and interdisciplinary team members.
  • Support transitions of care, including hospitalizations and returns.

7. Leadership & Communication

  • Serve as a clinical resource for staff, residents, and families.
  • Communicate effectively with the Director regarding clinical concerns, compliance risks, and staffing needs.
  • Participate in leadership meetings and contribute to department goals.
  • Promote a culture of accountability, compassion, and excellence in care.

Qualifications

Education & Licensure

  • Current, active Registered Nurse (RN) license in the State of Texas.
  • Graduate of an accredited nursing program.

Experience

  • Minimum of 1-2 years of nursing experience, preferably in:
  • Assisted Living (Type B)
  • Memory Care
  • Long-Term Care or Skilled Nursing.
  • Experience with RN delegation strongly preferred.
