As a Skilled Nursing RN at Neilson Place, you will manage a diverse patient population encompassing both long-term care and short-stay rehabilitation. Working the night shift, you will serve as a clinical leader, ensuring resident safety and well-being in a supportive, teamwork-driven atmosphere.
Key Duties:
Comprehensive Care Management: Assess, implement, and coordinate individualized plans of care, adjusting treatments as clinical needs evolve.
Clinical Excellence: Administer medications and treatments according to professional standards; perform wound care and monitor for therapeutic medication effects.
Physician Collaboration: Ensure all physician orders are processed accurately and communicate changes in resident status in a timely manner.
Resident Advocacy: Maintain resident self-esteem and privacy while providing opportunities for independence and personal choice consistent with their care plans.
Safe Mobility: Expertly utilize transfer equipment (gait belts, mechanical lifts, slide boards) and handling techniques to ensure residents feel secure and safe.
Documentation & Technology: Utilize the EMR accurately to document care provided and share vital information with the interdisciplinary team.
Education & Support: Respond promptly to call lights and provide essential health education to residents and their families or significant others.
Safety & Compliance: Adhere to all departmental safety, fire, and disaster plans while fulfilling obligations under the corporate compliance program.
Education: Graduate of an accredited school of nursing (ADN or BSN).
Licensure: Current Registered Nurse (RN) licensure for the state of Minnesota.
Certification: Basic Life Support (BLS) certification.
Clinical Skills: Knowledge of basic nursing skills and familiarity with the effects/side effects of administered medications.
Physical Requirements: Ability to lift at least 35 lbs and navigate the physical demands of a nursing home setting.
Communication: Must be able to effectively communicate in English, both orally and in writing.
Teamwork & Collaboration: A demonstrated ability to work effectively within a multidisciplinary team.
Clinical Judgment: Strong critical thinking and problem-solving abilities.
Compassion: A deep commitment to providing care in a patient, safe, and efficient manner.
Technical Expertise: Experience or proficiency in wound care is highly desired.
