Primary Job Responsibilities:
Conduct comprehensive patient assessments to evaluate their healthcare needs.
Develop, implement, and update individualized care plans based on patient assessments and preferences.
Administer medications and treatments as prescribed by the physician.
Monitor and document patient progress and responses to treatment.
Educate patients and their families on health management, medication administration, and care plans.
Coordinate care with interdisciplinary team members to promote optimal health outcomes.
Ensure compliance with agency policies, state, and federal regulations.
Act as a patient advocate, promoting their rights and preferences in care.
Requirements
Current RN license in good standing to practice in the Commonwealth of Pennsylvania.
Graduate of an accredited nursing school (BSN preferred).
At least 1 year of nursing experience; experience in home health preferred.
Strong clinical assessment and critical thinking skills.
Excellent interpersonal and communication skills.
Ability to work independently and manage time effectively.
Reliable transportation and valid Driver's License.
Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
Proof of Annual Influenza vaccination or an approved Medical or Religious exemption.
