A skilled nursing facility in eastern Massachusetts is seeking an experienced RN Pharmacy & Medication Coordinator for a full-time, non-bedside role offering $105,000-$110,000 annually with no clinical on-call responsibilities.
This position focuses on medication coordination, therapeutic interchange processes, and collaboration with pharmacy providers and physicians to support safe, efficient, and compliant medication management for residents.
Responsibilities
- Ensure medications align with facility formulary and medication profile
- Coordinate therapeutic interchange processes with pharmacy and physicians
- Facilitate prior authorizations and medication adjustments
- Communicate medication updates with the interdisciplinary care team
- Support compliance with medication-related regulations
- Maintain accurate and timely documentation
Qualifications
- Active RN license required
- Experience in skilled nursing, post-acute care, pharmacy coordination, or medication management preferred
- Strong organizational and communication skills
- Detail-oriented with the ability to manage multiple workflows
