Schedule: Full-time | Monday – Friday | 8 a.m. to 5 p.m.
Purpose:
Champion the delivery of outstanding patient care by overseeing and guiding clinical operations. Ensure healthcare services align with established standards and regulatory guidelines, fostering high-quality outcomes and continuous improvement within the community.
Primary Duties:
- Oversee clinical operations to uphold superior patient care standards, agency guidelines, and regulatory mandates.
- Lead the coordination and evaluation of patient care services.
- Guide and mentor the clinical team to ensure they deliver consistent, high-quality care.
- Maintain open and effective communication with patients, families, referral sources, and both field and administrative staff.
- Drive quality assurance through achievement of departmental goals and participation in quality enhancement projects.
- Exemplify professional conduct and uphold organizational values.
- Ensure compliance through involvement in necessary training, accurate documentation, and audit cooperation.
- Enforce a culture of safety by adhering to established safety measures.
- Facilitate staff development through guidance and opportunities for continued education.
Requirements:
- Valid registered nursing license for practice within the state.
- Previous experience in a home care environment with two years in a leadership or supervisory role is preferred.
- Strong understanding of professional standards and practices, Medicare guidelines, and relevant regulatory requirements.
