This Patient Care Manager role sits at the center of a home health operation — guiding clinical staff, overseeing patient care delivery, and ensuring every patient receives safe, high-quality services at home. You'll translate regulatory standards and agency priorities into daily clinical practice, directly shaping patient outcomes and the professionals who deliver care.
Your Day-to-Day
Provide clinical supervision to ensure patient care meets professional standards, agency policies, and applicable regulatory requirements
Coordinate and monitor home health care delivery across your assigned team
Supervise clinical staff to maintain consistent, high-quality service
Maintain clear communication with patients, caregivers, referral sources, and both field and office staff
Drive quality outcomes by meeting department goals and supporting performance improvement initiatives
Coach and mentor clinical staff, supporting their growth through continuing education and feedback
Uphold compliance through required training, accurate reporting, and cooperation with audits
Model professionalism and reinforce a culture of safety in every interaction
What We're Looking For
Registered Nurse with current, active licensure in Texas
Minimum two years of management or supervisory experience in a home care setting preferred
Working knowledge of Medicare Conditions of Participation, professional nursing standards, and federal/state/local regulatory requirements
