Position Summary::The Registered Nurse (RN) Care Manager or Licensed Social Worker (LSW/LISW) Care Manager works collaboratively as part of a care management team to onboard new families onto service and provide ongoing support to caregivers. This role is responsible for assessing consumer and caregiver needs, evaluating eligibility for service, and guiding prospective families through an efficient activation of services. The Nurse or Social Worker establishes a person-centric plan of care, completes an orientation to Careforth’s digital communication platform, and conducts regularly scheduled home visits. For an assigned caseload, this position coaches, advocates, and educates caregivers to assist with the daily health and well-being of their care recipients.

The pay range for this position is $61,248- $82,281. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
