Work Shift: Day Shift (United States of America) The pay range for this position is listed below. Primarily Monday-Friday, 8am-4:30pm with occasional weekends; Student Loan Repayment:  This position may be eligible for Avera’s Student Loan Repayment Program, which helps you pay off your loans and save money. Be part of a multidisciplinary team built with compassion and the goal of Moving Health Forward for you and our patients. Assists with coordinating the care and services of the patient diagnosed with cancer across the continuum of care in collaboration with multiple departments, specialties and provider disciplines. Promotes effective resource utilization, including removal of barriers to care. Assists with access to appropriate care through the multidisciplinary team in order to achieve an optimal clinical outcome. Facilitates the coordination of patient care services to assure excellence in patient care and patient flow. Follows patient through the entire care continuum/experience, eliminating operational barriers to services and addressing or making referral to address all other barriers to services or care compliance with particular focus on disparate populations. Works closely with other healthcare disciplines to ensure timely appointments, results reporting financial need referrals, communication, patient care, and follow-up. Documents all patient assessments, education, coordination, referrals, and other activity within the electronic medical record. Facilitates the development, implementation, and adherence to clinical and research guidelines to ensure optimal clinical outcomes. Ensures follow-up on identified patient clinical and non-clinical care, facilitating communication and compliance to established plans of care. Works in collaboration with the medical team to develop and implement clinical guidelines. Facilities and participates in multi-disciplinary care conferences or patient case review meetings and assists in follow up to ensure treatment plans are coordinated. Develops, organizes, and participates in educational activities related for patients and the community. Collaborates with other health care providers in developing, implementing, and evaluating educational materials for patients and their families. Interfaces with other healthcare team members for appropriate referrals/services (ethics, social work, fitness center, pastoral care, patient advocate, financial counseling, care management, community services, dietary, palliative care, speech therapy, etc.). Develops relationships with physicians and professionals within and outside of the immediate departments of oncology. Collaborates with outside disciplines and teams across physical locations to coordinate care into appropriate locations and within appropriate timelines and specialties. Assists Administration in planning, development, implementation and evaluation of goals and objectives including data collection, analysis, and reporting related to evaluation and measurement of impact due to the navigation process. Utilizes approved patient triage protocols, appropriately places patients with correct specialty, service, provider of location of care.
