SCH is seeking an RN/LPN Team Leader to join our dynamic Primary Care Team!! The Team Leader will have a passion for delivering the highest quality care to our patients. Additionally, the Team Leader will have solid leadership skills, with the ability to establish positive and collaborative relationships built on trust and respect. This position is primarily responsible for assisting and providing safe, consistent, and appropriate patient care within his or her scope of practice. Promotes and restores patient's health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. RN/LPN's role will include but not limited to: the assessment, planning, implementation, and evaluation of patients' statues, plan of care, and goal achievement, as well as supporting the RN Manager in the day to day operations of the clinic.
