JOB SUMMARY: Responsible for resident care using various nursing competencies such as assessments, care planning, implementation of interventions and evaluation of resident needs and care being provided. Support C.N.A.s to ensure resident care standards are being met and maintained. Must maintain confidentiality and adhere to HIPAA regulations.

MAJOR ACCOUNTABILITIES/ESSENTIAL JOB DUTIES:

  • Assess, implement, evaluate and/or re-evaluate resident care needs
  • Demonstrate ability and appropriate technical skills when administering treatment and procedures in accordance with physician orders, policies and regulatory requirements
  • Provide direction and assign daily duties to nursing staff
  • Act as a resource for staff concerning resident care and needs and keep management abreast of the status
  • Document pertinent information in resident's chart such as nursing interventions, any problems noted,

limitations of resident or significant change in resident's mood, behavior, affect or cognition

  • Meet all documentation standards and follow through with all charting requirements; ensure all C.N.A.s meet and maintain medical documentation data and worksheets
  • Use and adapt to new technologies, including tools, equipment, methods of application, computerized systems and processes, etc., to impact productivity of department and services; keep job-required technical skills up to date
  • Work collaboratively with the team and management to coordinate care and manage outcomes
  • Continue ongoing communication with resident, responsible party, physicians and others as appropriate
  • Inform nurse manager of any significant resident, family or staff issues or concerns
  • Additional duties as assigned

PHYSICAL REQUIREMENTS:

  • Ability to lift 25 pounds without assistance, 26+ pounds with manual or mechanical assistance
  • Ability to sit, stand, lift, bend, reach, push and pull to meet the needs of residents

REQUIRED KNOWLEDGE, SKILLS AND ABILITY:

  • Ability to make decisions and choices without direct supervision
  • Proficient verbal and written communication skills
  • Strong clinical assessment skills and clinical competencies

DESIRED KNOWLEDGE, SKILLS AND ABILITY:

  • Ability and knowledge to care for geriatric or cognitively impaired residents

REQUIRED EDUCATION AND EXPERIENCE:

  • High School Diploma or equivalency
  • Valid license as a Registered Nurse in the State of Virginia in good standing

DESIRED EDUCATION AND EXPERIENCE:

  • One year experience working with geriatric population
  • Experience working with cognitively impaired residents
  • Three years nursing experience

REQUIRED BEHAVIORAL COMPETENCIES:

  • Excellent customer service and interpersonal skills
  • Ability to make sound decisions based on objective analysis of a situation
  • Skill to proactively recognize situations where change is needed and to initiate appropriate action
