Responsibilities
Provide in-home nursing care and clinical assessment to patients.
Develop and update patient care plans with measurable goals.
Monitor and document patient progress; adjust care as needed.
Ensure compliance with safety, privacy, and regulatory standards.
Maintain accurate, timely documentation in electronic health records.
Coordinate with physicians and interdisciplinary team for care decisions.
Educate patients and families on treatment, medications, and safety.
Utilize data and pipelines to identify high-potential patients and optimize visits.
Meet or exceed quality and productivity targets through efficient scheduling.
Demonstrate professional development; pursue ongoing learning opportunities.
Engage in community outreach to facilitate referrals and partnerships.
Experience/Qualifications
Current RN license and BLS certification in home health settings.
Proven proficiency with electronic health records and home health documentation.
Ability to develop and update individualized care plans with measurable goals.
Strong clinical assessment skills with regular patient progress documentation.
Effective in coordinating interdisciplinary team and physician communications.
Experience with risk management, safety, privacy, and regulatory compliance.
Demonstrated achievement of quality and productivity targets under pressure.
Commitment to ongoing professional development and learning opportunities.
Data-driven approach to identify high-potential patients and optimize visits.
Strong patient and family education skills on medications and safety.
