PeaceHealth is seeking a RN - Home Health for a Per DiemRelief 0.00 FTE Day position. The salary range for this job opening at PeaceHealth is 42.77 - 68.68. The hiring rate is dependent upon several factors including but not limited to education training work experience terms of any applicable collective bargaining agreement seniority etc. This position is represented by a collective bargaining agreement. There may be more than one opening on this posting.Job SummaryResponsible for providing direct skilled nursing care and coordinationmanagement of patients admitted to home careclinic services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment problem identificationplanningintervention and clinicalgoal outcome evaluation.Essential FunctionsPerform initial evaluation of patienthome and apply admission criteria to determine if patient is safe candidate for home healthhospice services.Provide skilled nursing care to patients and skilled educationinstruction to patients and caregivers according to the home healthhospice plan of treatment.Performs physical assessments develops coordinate modifies and manages written individualized plan of care as ordered by the primary physician to meet patients long and short-term medical goals and appropriatetimely home health outcomes.Develops and clinically manages the Home Health Aides care plan according to the patients evolving personal are needs.Coordinates care collaborates and communicates with other members of the interdisciplinary team to assure appropriate and smooth flow of careservices through the care continuum.Performs discharge planning appropriately towards patient specific goalsoutcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.Provide accurate complete and timely clinical documentation in compliance with regulatory quality and Agency guidelines.Assist with the plan and implementation of family Bereavement services for the following patient death.Performs other duties as assigned.QualificationsEducationAccredited School of Nursing Required Nursing andBachelors Degree Preferred Bachelor of Science in Nursing. Experience2 years Required Acute clinical or home healthhospice nursing experience and 1 year Required Home health experience preferred or will orient to point of competency and Required Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patients needs CredentialsRequired Registered Nurse in state of practice andRequired Drivers License and proof of auto insurance andRequired Upon Hire Basic Life Support SkillsBasic computer skills including MS Office applications Required Well organized flexible and able to function independently Required Cooperative working skills leadership skills and interpersonal skills and ability to work effectively with a wide variety of patients families and staff Required Department Location Specific Notes PHOR and SIUSLAWRN Home Health job classification in ONA bargaining unit. SEARMinimum two years nursing experience required. One-year Home Health experience preferred or will orient to the point of competency.Nurses may at the Medical Centers option be required to float as needed to unitsMay be required to float to PHMG Clinics to provide coverage as needed Home and CommunityWill accept valid drivers license in US or Canada and proof of auto insurance.Working ConditionsLiftingPatient handling no greater than 35 lbs. without the use of assistive equipment andor devices NIOSH.Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.PushPull Frequently up to 45 lbs. force i.e. WOW medical carts.Perform hands on CPR at least 20 mins using 100-125 lbs. of force National Assoc. of EMS Physicians and AHA Frequency should be seldom or occasionally.Ability to move around area with frequent sitting.Bending stooping squatting reaching kneeling frequently.Lifting non-patient up to 20 lbs. occasionally and 5 lbs. frequently.Environmental ConditionsExposure to biohazard body fluids and airborne particles.Must be able to complete tasks in a noisy environment.MentalVisualVision and hearing required within normal limits glasses contacts hearing aids permitted.Ability to communicate and exchange accurate information.PeaceHealth is committed to the overall wellbeing of our caregivers.
