JOB RELATIONSHIPS

Responsible to the Agency Director. Consults with members of Home Health team as appropriate.

DEFINITION OF POSITION

Responsible for the continuous improvement in assessing, developing, and delivering a well-organized plan of care as it relates to nursing services and case management.

Supervises coordination of services and care for clients accepted for Home Health and Hospice and ensures the plan of care is developed, implemented, and updated.

Responsible for management and development of clinical staff.

Responsible for working within the scope of the Nurse Practice Act regarding judgment, nursing skills, education, and experience.

Clinical Operations Manager is available during all operating hours for staff and patient needs.

QUALIFICATIONS

Professional Current license to practice as an RN in the state of North Carolina.

Knowledge of current trends and techniques in nursing appropriate to practice setting.

Experience working as an RN within the last three years, with at least two years clinical experience in Medical/Surgical Nursing or Public Health/Home Health/Hospice setting preferred.

Knowledge of coverage issues and reimbursement regulations affecting Home Health preferred.

Current BLS Healthcare Provider Certification.

Maintain a valid North Carolina Drivers License, auto liability insurance, and dependable transportation.
