Key Responsibilities
Complete comprehensive patient assessments in stable and emergent situations
Develop, implement, document, and evaluate the Plan of Care (POC)
Maintain accurate, timely clinical documentation and patient records
Provide patient/family education (medications, home safety, self-care, etc.)
Coordinate with physicians, team members, and other healthcare professionals
Supervise/guide home health aides as needed
Follow infection control and safety standards (OSHA)
Support compliance with third-party payer requirements (as applicable)

Location: Bangor, ME 04401 (On-site/field-based home visits)
Hours: 40 hours/week
On-call: Required (may include evenings/weekends)
Productivity expectation: 30 units/week
Driving: Local travel required (reliable transportation + driver’s license + insurance)
