As a team member of our Senior Community Care SCC team we work to provide care to patients at home nursing homes and assisted living for senior housing. You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The RN is a flexible team member who works alongside nurse practitioners NPs and physician assistants PAs.
Standard Hours Monday-Friday 800 a.m. to 500 p.m. EST
Primary Responsibilities
Responsible for collaborating with all members of the interdisciplinary care team around changes in condition andor care coordination
Triage member needs to the appropriate care team member and level of care
Assist with transitions in care outpatient and inpatient
Assist in assuring complete and accurate documentation medication reconciliation nursing rounds and risk assessments
