Job Summary and Qualifications

As a Registered Nurse for Admission and Discharge, the top priority is delivering exceptional patient-centered care. They are dedicated to ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings. Specialized assessment skills help ensure that patients are placed in the appropriate care areas and discharged. We offer education and post-hospitalization care as necessary. Their main focus is advocating for patients, families, and caregivers while embodying the organizations vision, mission, and values. The goal is to give an unmatched patient experience and attain optimal clinical outcomes.

What will you do in this role?

  • Obtains a nursing assessment history and ensures proper inpatient care unit admission.
  • Takes appropriate action when needed, such as communicating special circumstances to physicians and/or care team or reporting indications of abuse or neglect.
  • Completes and facilitates the completion of required documentation, such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
  • Teaches patients, families, caregivers about a patients medical condition, patient status, treatment plan, medications, and possible side effects and follow-up measures.
  • Translates complex medical terminology to ensure complete knowledge.
  • Completes extra discharge documentation, education, and discharge care for special patient populations to ensure compliance with core measures for discharge.
  • Ensures patients, families, caregivers receive copies of discharge paperwork and any other documentation needed, such as referrals for ongoing care, educational materials, etc.
  • Coordinates with House Supervisors and Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
  • Demonstrates a time-conscious awareness and ability to prioritize, distinguishing between low and high-priority activities.

Qualifications

  • Basic Cardiac Life Support must be obtained within 30 days of employment start date
  • RN Registered Nurse
  • Bachelors Degree or Registered Nurse Diploma
