POSITION TITLE: RN, Developmental Disabilities (IDD) Nurse Case manager REPORTS TO (TITLE): Manager of IDD Services/Nursing Supervisor SALARY RANGE: $51.39 - $64.41 hourly DOE
Job Summary: The RN, IDD Nurse Case Manager is responsible for direct, indirect patient care and services coordination. S/he participates in the case management and care coordination of individuals with developmental disabilities; monitoring referrals, managing prescription refill requests. S/he is an integral part of the IDD care team focusing on positive outcomes for patients.
