Overview
To be responsible for providing oversight of all patient care services. Oversight includes making patient and direct care staff assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed and assuring the development, implementation and updates of the individualized plan of care.
Responsibilities
PRINCIPAL DUTIES AND RESPONSIBILITIES(*Essential Functions) Assists with the orientation process of new employees. Conducts supervisory visits of field staff members. Completes review of open and closed charts quarterly, and as needed. Reviews with manager and staff. Utilizes findings to formulate educational improvement plans with staff. Develops and implements performance improvement projects annually in collaborations with the manager. Assists in annual review of policies and procedures, and as needed establishment and revision of policies and procedures. Collaborates with manager for OASIS integrity, education, OASIS scrubbing reports, and review of payer requested charts. Collaborates with Home Health multidisciplinary staff, referral sources, patients and caregivers to promote quality care. Collaborates with Administrator/Manager to represent Home Care in the community and in hospital activities. Seeks opportunities for self-development through networking, peer collaborations, involvement in professional organizations, and other related education offerings. Performs other duties as needed and/or assigned.
