We are seeking a strong and compassionate Charge Nurse (RN) to provide frontline clinical leadership within our home and group home settings. Position Overview

The Charge Nurse is responsible for overseeing daily clinical operations, supporting nursing staff, coordinating schedules and staffing coverage, ensuring compliance with clinical standards, and serving as the on-shift clinical leader. This position combines leadership responsibilities with direct resident care and operational oversight to ensure continuity of care across assigned homes and programs.

The Charge Nurse plays a critical role in onboarding and developing clinical staff, maintaining quality assurance standards, and supporting residents requiring specialized ventilator, tracheostomy, and medically complex care.

Reports to: Director of Nursing (DON)

Key Responsibilities

Clinical Leadership & Staff Coordination

  • Supervise daily clinical operations and serve as the clinical leader during assigned shifts, including weekends as needed
  • Manage real-time staffing needs, callouts, and shift coverage coordination
  • Create and maintain clinical staff schedules
  • Support onboarding, orientation, and mentorship of newly hired nursing staff
  • Conduct skills validations, competency assessments, and performance observations
  • Assist with clinical interviews and provide hiring recommendations to leadership
  • Prepare updates, concerns, and discussion topics for staff meetings and leadership review

Resident Care & Quality Assurance

  • Ensure residents receive safe, high-quality, resident-centered care aligned with physician orders and care plans
  • Conduct chart audits to ensure documentation accuracy, compliance, and completion
  • Escalate complex resident care concerns appropriately to the Director of Nursing
  • Monitor authorizations, schedules, and continuity of care for assigned residents
  • Serve as a clinical resource for tracheostomy, ventilator, G-tube, and medically complex care needs

Medication & Clinical Oversight

  • Support medication management processes, including medication updates, narcotic audits, and refill coordination
  • Ensure PRN medication orders contain appropriate parameters and documentation follow-up
  • Conduct monthly medication cart checks and support medication delivery coordination
  • Monitor compliance with medication administration standards and documentation requirements

Supplies, Equipment & Resident Logistics

  • Oversee availability of medical supplies, DME, respiratory equipment, and resident care resources
  • Coordinate transportation and logistical arrangements for resident appointments and offsite care needs
  • Ensure emergency equipment and clinical supplies are maintained and accessible

Documentation, Compliance & Communication

  • Respond to clinical calls, voicemails, and communication through the Charge Nurse phone line
  • Track and complete assigned operational and clinical tasks through ClickUp and other systems
  • Complete incident reporting and escalate concerns appropriately in coordination with the DON
  • Ensure timely updates to Plans of Care (POCs), physician orders, eFax documentation, and EMR systems
  • Maintain compliance with HIPAA, infection control standards, APD requirements, and organizational policies

Direct Clinical Support

  • Provide direct nursing care, assessments, and treatments as needed
  • Complete monthly APD Monitoring Visits and required documentation
  • Support nursing staff with clinical guidance, emergency response, and problem-solving during shifts

Qualifications

Education

  • Associate or Bachelor’s Degree in Nursing required (BSN preferred)
  • Active, unencumbered Registered Nurse (RN) license in the State of Florida

Experience

  • Minimum of three (3) to five (5) years of nursing experience preferred
  • Minimum of one (1) to two (2) years in a supervisory, leadership, or Charge Nurse role preferred
  • Experience in home health, long-term care, LTAC, subacute, developmental disabilities, or group home settings strongly preferred
  • Experience with ventilator and tracheostomy care strongly preferred
  • Experience with EMR systems and clinical documentation standards required

Certifications

  • Current BLS certification required
  • ACLS certification preferred
