Required Skills:
  • Current RN license in the state of practice
  • Minimum of 2 years of related nursing experience
  • Strong understanding of care management policies, procedures, and evidence-based practices
  • Proficiency in using computers, electronic health records, and web-based applications
  • Excellent verbal and written communication skills
  • Good organizational and time management abilities
  • Effective conflict resolution and decision-making skills
  • Ability to adapt communication and care strategies to diverse patient populations

Nice to Have Skills:
  • Certification in Case Management (CCM) or Healthcare Management (CPHM)
  • Experience in healthcare coordination, discharge planning, or utilization review
  • Knowledge of federal, state, and local healthcare laws and accreditation standards

Preferred Education and Experience:
  • Registered Nurse Diploma required; Associates or Bachelor’s degree in Nursing preferred
  • At least 2 years of relevant nursing experience, ideally in case management or care coordination roles

Other Requirements:
  • Current Basic Life Support (BLS) certification from the American Heart Association
  • Willingness to work a 12-hour shift schedule (12:00 AM - 12:00 AM) as per operational needs
  • Ability to work in a physically demanding environment involving lifting and transportation of medical objects
  • Commitment to compliance with all safety, privacy, and regulatory standards
  • This role may involve work in areas with exposure to communicable diseases and hazardous materials, with appropriate safety precautions
