Required Skills:
- Valid RN license in the state of practice
- Minimum of 2 years of relevant nursing experience
- Proficiency in healthcare software, computer systems, and web-based applications
- Strong communication skills, both verbal and written
- Excellent organizational and time-management abilities
- Sound clinical judgment and decision-making capabilities
- Knowledge of care management policies, evidence-based criteria, and levels of care
- Ability to adapt communication styles to diverse patient populations
Nice to Have Skills:
- Certification in Case Management (CCM) or Healthcare Management (CPHM)
- Experience with patient discharge planning and care coordination
- Bilingual abilities or multilingual capabilities
- Familiarity with healthcare compliance and regulatory standards
Preferred Education and Experience:
- An Associates or Bachelors degree in Nursing is preferred
- At least two years of nursing experience, preferably within case management or similar settings
Other Requirements:
- Current Basic Life Support (BLS) certification from the American Heart Association
- Willingness to perform in a healthcare environment with potential exposure to communicable diseases
- Ability to work flexible hours, including shifts as assigned
- Commitment to maintaining compliance with all federal, state, and local healthcare regulations and standards
